Problem Statement: Recurrent pregnancy loss (RPL) classically refers to the occurrence of three or more consecutive losses of clinically recognized pregnancies prior to the 20th week of gestation. It´s incidence is 0.4 to 1%. After evaluation, it remains unexplained in approximately one-half of couples and therapeutic recommendations are largely based upon clinical experience and data from observational studies. Hence, it is one of the most challenging areas in reproductive medicine.
Case report: 33 years, IO 0040, referred to our Maternal-Fetal Department at 8 weeks of gestational age. Prior evaluation had not revealed a plausible cause for her primary RPL. Pregnancy was managed with folic acid 5 mg id, aspirin 100 mg id and progesterone 200 mg tid. First trimester screening was normal. At 12 weeks Ureaplasma infection was diagnosed and treated. Pregnancy developed without other intercurrences until 17 weeks when she was admitted in the Emergency with painless rupture of membranes. Autopsy of the fetus revealed no alterations.
Conclusion: RPL is an important problem in women's health since etiology is often unknown and there are few evidence-based diagnostic and treatment strategies. Despite the advances, many questions regarding etiology, evaluation, and management of these couples remain unsolved.
